The type of parental responsibility you have for your child is not based upon who your child lives with, but on other things such as when and where your child was born, whether or not you were married to the other parent at the time of the childÕs birth or at any time since, and whether the fatherÕs name is on the birth certificate.
You will be required to select the type of parental responsibility you have on the application form below.
If you have sole parental responsibility for your child you will not need the other parentÕs signature on your childÕs Deed Poll. Your signature alone will be enough.
If you share joint parental responsibility, your childÕs Deed Poll will require the other personÕs signature in addition to your own.
Without the correct consent, your childÕs Deed Poll may not be accepted by official UK organisations such as the Passport Service.
